External adapters package.

The adapters package in core holds integrations with external tools. Its one subpackage, claude, adapts Claude Code: a tool interceptor that applies governance to each tool call, a hook orchestrator that runs configured hooks by category and priority, and the failure-policy declarations for governance gates. The package root re-exports the main classes and provides lazy accessors for them. Nothing else imports this package at runtime except the governance gate script; the tests in tests/core/test_adapters.py exercise it. Read the claude index first.
