The decision phase of the factory. phase (299 lines, a residue-only lift under ADR-0103 P2-F) runs the phase using decision_inputs, and records the outcome as decision_record, blocking_item and ratification_item. design_authority is the interface for who decides, with l_l_m_design_authority as the LLM-backed implementation, and detector_engines supplies the detector engines it consults (detectors).
