RPC handler modules -- split from the monolithic RPCHandler.

The RPC handler modules of the gateway, split out of the former monolithic RPCHandler, so that each RPC method group has its own module. They are dispatched from the method_table of orchestrata_gateway. By subject: sessions (session_handler, session_kg_ops, session_event_ops), the kernel and orchestration (orchestrator_handler, cxp_handler, workflow_handler, volk_handler), governance (governance_handler, policy_gate, scope_guard), knowledge and state (knowledge_handler, memory_handler, state_handler, projects_handler, config_handler), gateway administration and authentication (gateway_handler, auth_handler), tests (test_metrics_handler), and the integration handlers fireflies_handler and ultimate_t_u_i_handler. utils holds shared helpers. The largest are cxp_handler, volk_handler, session_handler, knowledge_handler, gateway_handler and projects_handler, each about 400 lines or more.
