Context

Mohamed asked of round-16 data: "where is it documented? does it enrich the knowledge graph? does it enrich the wiki?" Honest state when asked: the design's §12 ledger, the status doc, and all agent briefs were committed, but the 39 full evidence files (two lens reports per round, every fold-verifier report) lived ONLY in the volatile session scratchpad, and nothing from the review stream reached the wiki or the graph.

Decision (proposed)

Three homes for review evidence, mirroring ADR-0046's decisions rule: (1) REPO — every round's reports + verifier files committed to plans/reviews// the turn they land (rounds 1-16 backfilled: 39 files / 27,210 lines, commit 3e6eb9c5, with a scoped gitignore exception); (2) WIKI — a per-design review-index page on the knowledge-wiki (rounds, counts, worst findings, readiness deltas, links), appended per round; (3) GRAPH — the evidence ingested into the Memgraph semantic twin as ArchitectureChunk documents (the mechanism the ADRs + design docs already use), so sixteen rounds of findings are semantically retrievable beside the decisions they produced; the operational KG links design-version nodes to their round evidence via the existing artifact-provenance path.

Alternatives rejected

Scratchpad-only (volatile — the state Mohamed caught); repo-only (invisible to graph queries and wiki readers — the exact gap ADR-0046 closed for decisions).

Consequences

Review evidence becomes durable, citable, retrievable; each round's fold routine gains two mechanical steps (wiki index append + semantic ingest).
