The record
Status: Accepted · Date: 2026-08-29
Context
Decisions were scattering: docs/adr held the SPEC-033 series, the knowledge-wiki decisions page held the architecture-ontology series, the graph held ArchitectureDecision nodes, and the ruleset-design decisions initially existed only in session prose. Mohamed's ruling, verbatim: "ADR must be in graph, wiki and docs."
Decision
Every ADR is recorded in all three homes, same title, same status: (1) docs/adr/ NNNN-kebab-title.md — the append-only file of record; (2) the knowledge-wiki per-project decisions.md page — the human register (## ADR-NNNN: Title sections); (3) an ArchitectureDecision node in the operational graph, written via sync_architecture_to_graph.py --adrs. check_adr_graph_sync.py is the standing three-way drift gate; a new ADR is not DONE until the gate is clean for it.
Alternatives rejected
Single-home (docs only): invisible to graph queries and wiki readers. Manual mirroring without the gate: proven to drift (this very ruling was triggered by drift).
Consequences
ADRs 0039-0046 are the first batch recorded under this rule; the decision-recording workflow gains a mandatory gate step.
Provenance
Mohamed's directive, decision session 2026-08-29.
