Purpose
Post-reboot verify (suite green) + backlog #4 Neo4j repo seam swarm-refactor
Work log
- Reviewed+hardened the uncommitted per-invocation COVERAGE_FILE fix: added _purge_stale_batch_coverage (run-start purge of stale .coverage.batch* ONLY when --cov active) + 2 unit tests (17 pass) at 2026-07-08 17:42
Details:
- uuid-suffixed batch files from aborted/uncombined runs would silently inflate the next coverage combine; purge gated on --cov so plain re-runs never delete data awaiting combine
- Backlog #163 design: full driver-usage catalog (23 files, sync/async/exotic) via Explore audit; wrote plans/neo4j-seam-design.md — core/graph/ package, Sync/Async GraphRepository protocols + UnitOfWork, ownership semantics, escape hatch, 7-phase per-subsystem migration order at 2026-07-08 17:42
- Fixed both post-reboot suite failures (742K-import fallout): memgraph adapter consume() on all 9 write paths (silent failure swallow, verified live), CASE-guarded clear_test_data (Memgraph AND doesn't short-circuit), Entity lookup indexes (>120s timeout -> 8.79s), ontology map_node canonical-key normalization (root cause of the 2 integer-id nodes) at 2026-07-08 18:00
- Seam Phase 0 landed (17e34088): core/graph package via Sonnet builder, reviewed by me — fixed uow exception-masking + free-threaded lazy-driver race (+ regression test); 53 tests, 100% cov, all gates. 3 scoped commits: 72a83521, bf82fd1e, 17e34088 at 2026-07-08 18:00
- HA incident: post-reboot replication frozen (both replicas behind=-131, ts stuck, status 'ready' — controller blind to lag). User-approved DROP+REGISTER on MAIN -> both replicas recovered to behind=0 @ ts 428153, 762,370 nodes; integer-id fix verified on all 3 nodes. Controller consume()+idempotent set-role fix committed (exposed 7 false-confidence HA test passes) at 2026-07-08 18:10
- HA controller freeze-detection landed: _note_replica_progress tracks per-replica (ts, frozen-cycles); 'ready' + data outstanding + non-advancing ts for 10 poll cycles -> forced re-registration ('recovery' exempt). 8 pure state-machine tests; 31/31 HA module green. Commits: controller consume/idempotent-set-role + freeze detection at 2026-07-08 18:14
- Phases 1+2 of the seam migration landed (edf93ab6, 460b6237, 7db35929): gateway (server/rpc/5 handlers/test-runner) + 7 async internals on AsyncGraphRepository; driver_kwargs added to config for timeout parity; 2 latent defects fixed (engine health_check stale attr, resolver raw-driver bypass); pyright 302->297; fixed 4 full-suite stragglers the targeted runs missed at 2026-07-08 18:55
- Full suite GREEN after phases 1+2: 22708 passed / 0 failed / 0 errors, 1156/1156 modules, 7.0m. Phases 3 (postman vestigial cleanup) + 4 (sync files incl. cxp_auditor uow + postman.py pass-through) launched in parallel at 2026-07-08 19:03
- Phase 3 landed (64aa67eb): vestigial Neo4j handles removed from postman receipt/replay/message stores; fixed the 'try: pass' Redis probe that hardcoded REDIS_AVAILABLE=True (hidden-red slow test now passes). Builder had staged the shared index mid-flight — reset before scoped commit (clean-index rule held) at 2026-07-08 19:17
- User caught QueryMixin missing from the migration: root cause = phase-0 inventory scanned import-neo4j/create_neo4j sites, blind to attribute-coupled driver use. Sweep of .session( found 6 missed files (QueryMixin, context_injector, context_pack frame+selector, fireflies_sync, canonical_schema) — added as phase 5b in the design doc; phase-6 gate must ban .session( outside the seam, not just imports at 2026-07-08 19:36
- Phase 4 committed (bed9e2f8, full suite 22702/0). Phase 5+5b launched as two parallel builders: A = coupled cluster (context_injector, context_pack frame/selector, pipeline_kg reach-through removal, synchronizer dual-acquisition, QueryMixin, sync.py compat-property deletion); B = independents (kg_context uow, kg_sync/_base escape hatch, fireflies_sync, canonical_schema + kg_schema CLI) at 2026-07-08 19:41
- Tracing swarm launched (user directive): 18-packet Workflow, map->adversarial-verify pipeline over 869 untraced defs + 924 dangling @traces sites (721 = phantom REQ-100-*). Ground truth staged from the authoritative specs: 4864-requirement index; packets exclude phase-5 in-flight files. Rules: no id outside the index, unmapped = spec-gap finding (never fabricate), check-traces + import gates per packet at 2026-07-08 20:30
- Phase 5/5b both parts delivered (uncommitted, interleaved with live tracing swarm): part A killed the ContextInjector._driver reach-through (pipeline_kg owns one repo), migrated context_pack+synchronizer+QueryMixin, deleted the _driver compat property; part B did kg_context/kg_sync-escape-hatch/fireflies_sync/canonical_schema. context_injector 81% = known retired-handler debt (P3 deletion backlog). Part A also caught+fixed llm/config.py missing traces import from the swarm; other 9 flagged files self-healed via swarm gates at 2026-07-08 21:13
- CP1 suite triage: fixed a real part-A bug (context_injector PEP-649 lazy annotation NameError — SyncGraphRepository import hoisted from TYPE_CHECKING to runtime), 3 signature tests updated driver->repo (intentional seam API), 2 placeholder-pinning tests rewritten to durable invariants (they asserted REQ-000-000 IS present — enshrined fabrication; now assert well-formed + placeholder-free), 3 missed TestResultsStore(driver=) constructors in test_real_neo4j_knowledge fixed. All 12 flagged modules green; full suite rerunning at 2026-07-08 22:18
- Runner defect FIXED and committed (ca50077f): root cause measured — i%48 round-robin packed 9 fast modules behind a 285s Neo4j-error-path module in batch 7, crossing the 273s graceful timeout; LPT packing from a duration cache + explicit did_not_run flag + one rerun round; 140 threading tests green, 98% file coverage. Runner-fix agent TaskStopped on acceptance at 2026-07-09 00:04
- Runner gap-2 fixed: modules SIGKILLed mid-test now rerun-eligible in SOLO bins at 2026-07-09 14:11
Details:
- live-log rescue keeps did_not_run+suspected_hang; _prepare_rerun_batches gives each suspected hang a solo bin so a real hang cannot poison shared rerun batches
- 53/53 executor tests pass; commit 32d48510
- Launched pytest-native-features audit swarm (wf_b332f148-faa) at 2026-07-09 14:11
Details:
- 6 research agents: core/reportlog, xdist deadlock forensics, pytest-split, rerun plugins, timeout/hang, free-threaded parallelism
- synthesis writes plans/pytest-native-audit.md
- pytest-native audit complete + adoptions committed (c7cffea6) at 2026-07-09 14:22
Details:
- 7-agent swarm -> plans/pytest-native-audit.md; adopted: -p no:xdist + no:cacheprovider per batch, faulthandler_timeout=per-test+30, pytest-timeout>=2.4.0 pin
- measured: grpc and janus_swi silently re-enable the GIL on 3.14t (isolated per-process check)
- W6 acceptance FAILED: 224/407 = 55.0% (gate 95%); 183 wrong decorators saved to plans/trace-acceptance-wrong.json at 2026-07-09 14:32
Details:
- judge quality verified by 2 direct spot-checks (peek/REQ-007-US-005-011 class D, handle_put/REQ-014-129 confirmed)
- root cause: W1-W5 were defect-class-driven; population mass never flagged by any detector retains fabricated ids
- worst strata: gateway 14.3%, kernel 29.5%, knowledge_graph 33.3%; best: harness/tools/openspec/refactor 100%, postman 94.7%
- W6.5 launched (wf_d37360aa-3ba): 154 fix+verify packets over 3,283 decorators, 18 failing strata at 2026-07-09 14:36
Details:
- packets grouped by file (386 files), each site carries top-8 OpenRouter RAG candidates; 200 sites carry their W6 judge verdicts
- brief: exhaustive per-id re-judgment, ~45% measured base rate, alias-class ruling added
- excluded strata (>=90% in W6): postman, harness, openspec, refactor, tools
- Suite hotspot analysis + fix: unbounded Neo4j managed-transaction retries (commit 873cee21) at 2026-07-09 14:43
Details:
- top-2 modules 438.5s -> 66.5s (76/76 pass); serial suite 52.9 min -> ~46.7 min
- mechanism: execute_read retries ServiceUnavailable for max_transaction_retry_time=30s default against dead addresses; bound to 1.0s
- remaining top: test_pipeline_kg 184.8s, test_orchestrata_tools 128.6s, test_coverage_queue_manager 103.4s, test_test_executor 83.3s (justified: spawns deliberately-hanging pytest subprocesses)
- Profiled the 3 slowest modules; fixed 2 (commits 917099b4, 9811dd51) at 2026-07-09 14:58
Details:
- test_pipeline_kg: 4th unbounded-Neo4j-retry instance, 100.7s -> 9.6s
- test_coverage_queue_manager: redis-py 7 default Retry(ExponentialWithJitterBackoff,3) = 5.3s/command vs dead port; QueueManager gained redis_client injection seam; 94.3s -> 2.1s
- test_orchestrata_tools: structural — 5 API entry points each re-run a full-codebase matching pass (~15s each); fix belongs in orchestrata_tools matcher memoization, deferred with numbers
- orchestrata_tools matcher memoization (commit ab192aa1): module 128.6s -> 20.5s at 2026-07-09 15:11
Details:
- identity-keyed bounded cache on (test_ast, source_index); 15.15s -> 0.03s cross-instance; copies returned, lock-guarded for 3.14t
- test fixtures rerouted through cached _source_index() singleton
- W8 prevention shipped (6bdd74ec): trace-lint spec-mismatch rule + deterministic provenance pass + pre-commit + conventions at 2026-07-09 19:21
Details:
- tree: trace-lint 0 errors, check-traces 0 dangling, 505/505 trace-asserting tests
- 2 tests updated that pinned pre-ruling state (alias-class, core/base infra-lane)
- Evidence-derived trace writer: citations→direct-call evidence→@traces at 2026-07-09 19:55
Details:
- add_traces_from_evidence.py joins REQ citations (docstring/comment only) to source defs via BodyMatcher.direct_call_index (new public API; fixture+decorator calls excluded, import-disambiguated); 25 new decorators + 18 merges across 22 src files, all membership-true, trace-lint 0 errors, 381 affected tests + 30 new tests green
- Full-gate pass over the evidence-trace change set; 2 real bugs fixed at 2026-07-09 20:17
Details:
- code-review.py tree-sitter path never ran (undefined _py_lang NameError swallowed); generate_rtm_evidence tokenize.TokenizeError->TokenError; mypy --strict 0 on changed files; TRACE_MISSING downgraded to WARNING + scoped off orchestrata_tools/scripts per two-lane ruling; suite 22795/0/0 in 336s
- orchestrata_ast extraction complete + BodyMatcher split steps 1-5 at 2026-07-09 21:08
Details:
- New stdlib-only src/orchestrata_ast (models/analyzer/serialize/inheritance) extracted from the 9.6k-line script; script re-exports; serialization byte-identical; orchestrata_tools + evidence writer consume orchestrata_ast.inheritance; local InheritanceMap deleted per ruling 'no hand-rolled inheritance checks, enhance the AST component'
- Fixed parallel-suite flake: PipelineEventBus events_key seam at 2026-07-09 21:26
Details:
- test_emit_with_redis counted on the hardcoded shared Redis list; 8-way parallel batches interleave; seam added (default pinned by test), fixture docstring's 'suite runs serially' premise corrected; committed
- Root-caused coverage combine data loss: relative COVERAGE_FILE resolved against grandchild cwd (tmp_path) — parts deleted with fixture dirs. Fixed with absolute anchor (b2a95525); relaunched instrumented full suite for a true baseline. at 2026-07-09 21:41
Details:
- All 16 zero-percent files confirmed measurement artifacts (probes 78-100% real)
- User directive: coverage waves until every src file >90%
- ROOT-CAUSED + FIXED the whole-batch coverage loss (bd88c205): conftest pytest_unconfigure's os._exit grpc-hang workaround skipped atexit, where patch-mode coverage saves parts — every grpc.aio-importing batch exited green with zero coverage. Fix: stop+save Coverage.current() before hard exit. Verified via PTE_COV_PART_LOG diagnostic (26/60 children parts=0 → killers now parts=1). at 2026-07-09 22:07
Details:
- Also isolated nested pytest-cov data files in test_session_coverage_gates (latent slow-lane part destroyer)
- Coverage source expanded to orchestrata_ast + orchestrata_tools + scripts (8dcbc9f9) per user ruling 'not just src'
- Run 4 (clean baseline) in flight
- Coverage wave 1 committed (94d6fc3e): ultimate+ast+tools all targets 92-100%; 628 tests green combined; 10+ real source bugs fixed incl. @traces propagation that NEVER worked in enhanced_generator, classmethod/async codegen bugs in diagram_to_code, runtime_main serve seam. Dead-scripts deletion committed (6a186c9d, 29 files ~6k stmts, user-approved). Wave 2 (5 agents) running on live scripts. at 2026-07-09 22:44
Details:
- src/orchestrata_ultimate truly 97.8% — the 156-file worklist was the os._exit measurement bug
- scripts audit: 35 live to test, 14 deleted, 4 unwired ruled TEST
- Wave-3b/3c committed: CLI wrappers + ops/gate scripts >=90% at 2026-07-10 00:11
Details:
- 8d6aa61f: rewired test-gen.py/review-tools.py (both crashed on every run — API drift), ast_api.py Path fix, 78 CLI tests
- f7a64c9f: agent_check mypy --output-format=json silent-zero bug + auto_fix LineLengthFixer discarded-split bug fixed, 132 tests, cluster 99%
- MemgraphSemanticGraph.clear() Entity-wipe removed per user ruling (uncommitted, rides with semantic-graph cluster)
- In flight: runner cluster, semantic-graph ingest >=90%
- Gate to 0 blocking + PYTEST_XDIST_WORKER renamed to ORCHESTRATA_WORKER_SLOT at 2026-07-10 00:52
Details:
- d50b2a95: gate-ok waiver channel (reason required) for 11 fixture-string false positives + 1 label-scoped delete; gate now exits 0
- 2aee8349: env var renamed repo-wide (user ruling — old name read as xdist); gw<N> format kept; agent defs, hook, docs, memories updated
- db4cf1c8/5f2d5ef4: runner + semantic-graph clusters landed; ingest 100%, clear() Entity-wipe removed
- Final instrumented full-suite run in progress
- Seam #163 phase 6 (final): D13 graph-seam gate committed (219b83c3) at 2026-07-10 01:43
Details:
- AST gate: neo4j imports + .session( calls banned outside seam/escape-hatch allowlist; probe pattern (bare import in try/except ImportError) sanctioned
- Caught 4 probe imports the grep inventory missed; checker green on 650 files; 11 tests, checker 97%
- Also: force-test-observation deny-hook (8b791903) + memory; wave-5 sweep found 11 files <90%, 4 test-writer agents in flight
- Rogue auto-commit 724b556e ('Session 400: Say hello...') reverted at 2026-07-10 01:55
Details:
- An external session's auto-commit swept 18 uncommitted in-flight files (finished wave-5c work + two agents' half-done files) into one commit
- git reset --mixed HEAD~1 restored the exact pre-commit tree; daemon roster stale, no live rogue process found
- Wave clusters recommitting individually after verification as planned
- Solo-retry round in run_tests.py + executor JUnit-attribution fix at 2026-07-10 04:07
Details:
- Capped solo-retry (default 5, --solo-retry-cap) re-runs batch-failed modules in isolation; both rounds reported, recoveries logged to .flake_recoveries.ndjson; exit reflects post-retry verdict
- Real executor bug found via the fixture: _dotted_module_map garbled absolute paths so whole batches read as did-not-run and double-ran via Phase-2B — fixed (relpath to cwd; out-of-tree = stem) + normalized file-attr matching
- force-test-observation hook runner regex tightened twice; 23-case matrix green
- Commits 4916e57a, fec5926b
- Drafted plans/reproducibility-plan.md (Task #9): live fleet audit + 6-phase plan at 2026-07-10 04:11
Details:
- Audited 23 Incus containers: redis-queue .10, neo4j-db .11 and ALL Memgraph HA nodes are DHCP-lease-only (no ipv4.address pin) — the load-bearing IPs are the unpinned ones; only 8/23 pinned
- Memgraph MAIN is .111 (failover from .192) — plan mandates resolve_main() via HA controller, never doc-pinned MAIN
- Phases R1 pins, R2 .env.example+validate_env, R3 MAIN discovery, R4 bootstrap_graphs (kg_schema+sync_ast+sync_architecture+enrich), R5 Terraform+incus provider with import-not-recreate + drift gate + rehearsal project, R6 CI parity via canonical runner; acceptance criteria + effort table
- RELEASE v1.3.0 pushed — session goal achieved at 2026-07-10 04:45
Details:
- master bc4edb87 (107 commits) + annotated tag v1.3.0 pushed via glab helper; verified on remote via ls-remote
- v1.2.0 was found already published at old commit 89bf5d11 (premature tag from an earlier session) — published tags never moved, released as v1.3.0 instead
- Coverage evidence: no-filter sweep 97.6% total / 74151 stmts, all first-party files >=90%; llm_integration-gated scripts measured explicitly (93%/94%) since default addopts -m excludes them from every sweep
- Solo-retry runner proved itself live: 3 full runs, each with 1-2 batch flakes, all auto-recovered solo, flake log accumulating (test_constraints_real.py now a 2x repeat offender)
- Memgraph→orchgraph migration: safe prep done, gated on kimi + sign-off at 2026-07-10 05:52
Details:
- Backup: 558M verified tar of source data dir. Discovery: source cluster shared (762376 nodes/497961 rels, 58 labels, orchestrata + knowledge-wiki co-mingled)
- IaC committed f2d49bc8: terraform+incus for orchgraph, validates clean, hardens the two live-cluster defects (data-dir /tmp->/var/lib persistent; DHCP->pinned IPs)
- Parity verifier built + baselined (the pre-decommission gate). Mohamed decision: full port both projects + decommission old, AFTER parity PASS + sign-off
- BLOCKED on kimi Step 1 (move orchgraph data-dir off /tmp) then Step 2 (snapshot/DUMP port); kimi async, only canned acks so far
- Built and deployed vector_search.so to orchgraph Memgraph HA cluster; unblocked RAG by enabling vector_search.search/search_edges/show_index_info procedures. at 2026-07-10 10:31
- Diagnosed root cause: orchgraph fork binary lacked query-modules-directory config and vector_search.so deployment; systemd units updated on orchgraph-data-1 and orchgraph-data-2. at 2026-07-10 10:31
Artifacts
- plans/neo4j-seam-design.md - plans/trace-remediation-plan.md (190 lines) - Trace remediation program: defect taxonomy D1-D9 (5061 decorators/14395 ids; fan-out REQ-002-001 x370; 199 shotgun decorators; 196 sequential-stamped files), waves W0-W9 with entry/exit criteria, statistical acceptance gate, downstream rebuild, prevention gates- plans/bodymatcher-split-plan.md (170 lines) - plans/prolog-utilization-plan.md - scripts/run_tests.py - src/orchestrata_ultimate/core/threading/test_executor.py - plans/reproducibility-plan.md - /home/mohamed/workspace/orchgraph/build/query_modules/vector_search.so - Built query module for orchgraph Memgraph 3.11 fork (Debug+MG_ENTERPRISE=ON)- /tmp/deploy_vector_search.sh - Deployment script that pushes vector_search.so and updates systemd units on orchgraph data nodes
Decisions
- Trace remediation runs as a staged program (plans/trace-remediation-plan.md), not ad-hoc rounds: User ruling: rounds 1-2 were reactive; census showed the real problem ~10x larger (default-stamping top-5 = 1371 occurrences, 199 shotgun, 196 sequential files) — needs instrumentation, serialized waves, fresh-eyes statistical acceptance, artifact rebuild, prevention gates at 2026-07-08 21:48
- Thread-based test distribution REJECTED for now; batched-subprocess executor stays: Measured: grpc + janus_swi re-enable the GIL on 3.14t so threads are serialized in the heaviest subsystems; per-worker isolation lives in process-global os.environ; hung tests are non-cancellable in threads (no killpg equivalent); no thread-distribution runner exists upstream (xdist#904/pytest#13768 unimplemented; pytest-run-parallel is a race stress validator, not a distributor). Watch items recorded in plans/pytest-native-audit.md at 2026-07-09 14:22
- Two-lane traceability: business code -> FRs; infra code -> explicit NFR or deliberately untraced: User ruling 2026-07-09: infra serves many purposes; force-fitting business reqs onto utilities is the fabrication engine (RingBuffer.peek/autonomy-verification). Codified in plan §2.5 + live w65_brief.md; W9 must separate untraced-infra(by design) from untraced-business(spec gap) at 2026-07-09 14:50
- Trace program FROZEN at prevention-only after strategy review; W6.5 stays stopped: W1-W5 wave QA (0.6-12.5% wrong) vs W6 corpus measurement (45% wrong) = non-convergence; 95% gate set without measuring inter-judge agreement; no named consumer of the corrected corpus. Deterministic replacements shipped in 6bdd74ec: spec-mismatch lint rule (found on 55% of judged-wrong decorators), 118-decorator provenance normalization (15 rewrites + 103 splits), pre-commit gate, AGENTS.md conventions, 55%+/-5% corpus caveat at 2026-07-09 19:21
- check-traces ratchet re-baselined 236→2219: code_reqs dropped 4654→2636 distinct ids via deliberate W4/W5/W6.5 removal of measured-wrong claims (-1331 pre-b4ae35e5, -687 in b4ae35e5); old floor rested on 55%-wrong decorators; 2219 untraced is the honest count at 2026-07-09 19:55
- Deployed /opt/memgraph/query_modules/ instead of /usr/lib/memgraph/query_modules because orchgraph binary lives at /opt/memgraph/memgraph: Container inspection showed /usr/lib/memgraph does not exist; /opt/memgraph is the actual deployment prefix. at 2026-07-10 10:31
- Built vector_search.so in existing /home/mohamed/workspace/orchgraph/build tree: Claude verified deployed /opt/memgraph/memgraph is byte-identical to build/memgraph, so ABI/config match is guaranteed. at 2026-07-10 10:31

Purpose
Post-reboot verify (suite green) + backlog #4 Neo4j repo seam swarm-refactor
Work log
- Reviewed+hardened the uncommitted per-invocation COVERAGE_FILE fix: added _purge_stale_batch_coverage (run-start purge of stale .coverage.batch* ONLY when --cov active) + 2 unit tests (17 pass)
- uuid-suffixed batch files from aborted/uncombined runs would silently inflate the next coverage combine; purge gated on --cov so plain re-runs never delete data awaiting combine
- Backlog #163 design: full driver-usage catalog (23 files, sync/async/exotic) via Explore audit; wrote plans/neo4j-seam-design.md — core/graph/ package, Sync/Async GraphRepository protocols + UnitOfWork, ownership semantics, escape hatch, 7-phase per-subsystem migration order
- Fixed both post-reboot suite failures (742K-import fallout): memgraph adapter consume() on all 9 write paths (silent failure swallow, verified live), CASE-guarded clear_test_data (Memgraph AND doesn't short-circuit), Entity lookup indexes (>120s timeout -> 8.79s), ontology map_node canonical-key normalization (root cause of the 2 integer-id nodes)
- Seam Phase 0 landed (17e34088): core/graph package via Sonnet builder, reviewed by me — fixed uow exception-masking + free-threaded lazy-driver race (+ regression test); 53 tests, 100% cov, all gates. 3 scoped commits: 72a83521, bf82fd1e, 17e34088
- HA incident: post-reboot replication frozen (both replicas behind=-131, ts stuck, status 'ready' — controller blind to lag). User-approved DROP+REGISTER on MAIN -> both replicas recovered to behind=0 @ ts 428153, 762,370 nodes; integer-id fix verified on all 3 nodes. Controller consume()+idempotent set-role fix committed (exposed 7 false-confidence HA test passes)
- HA controller freeze-detection landed: _note_replica_progress tracks per-replica (ts, frozen-cycles); 'ready' + data outstanding + non-advancing ts for 10 poll cycles -> forced re-registration ('recovery' exempt). 8 pure state-machine tests; 31/31 HA module green. Commits: controller consume/idempotent-set-role + freeze detection
- Phases 1+2 of the seam migration landed (edf93ab6, 460b6237, 7db35929): gateway (server/rpc/5 handlers/test-runner) + 7 async internals on AsyncGraphRepository; driver_kwargs added to config for timeout parity; 2 latent defects fixed (engine health_check stale attr, resolver raw-driver bypass); pyright 302->297; fixed 4 full-suite stragglers the targeted runs missed
- Full suite GREEN after phases 1+2: 22708 passed / 0 failed / 0 errors, 1156/1156 modules, 7.0m. Phases 3 (postman vestigial cleanup) + 4 (sync files incl. cxp_auditor uow + postman.py pass-through) launched in parallel
- Phase 3 landed (64aa67eb): vestigial Neo4j handles removed from postman receipt/replay/message stores; fixed the 'try: pass' Redis probe that hardcoded REDIS_AVAILABLE=True (hidden-red slow test now passes). Builder had staged the shared index mid-flight — reset before scoped commit (clean-index rule held)
- User caught QueryMixin missing from the migration: root cause = phase-0 inventory scanned import-neo4j/create_neo4j sites, blind to attribute-coupled driver use. Sweep of .session( found 6 missed files (QueryMixin, context_injector, context_pack frame+selector, fireflies_sync, canonical_schema) — added as phase 5b in the design doc; phase-6 gate must ban .session( outside the seam, not just imports
- Phase 4 committed (bed9e2f8, full suite 22702/0). Phase 5+5b launched as two parallel builders: A = coupled cluster (context_injector, context_pack frame/selector, pipeline_kg reach-through removal, synchronizer dual-acquisition, QueryMixin, sync.py compat-property deletion); B = independents (kg_context uow, kg_sync/_base escape hatch, fireflies_sync, canonical_schema + kg_schema CLI)
- Tracing swarm launched (user directive): 18-packet Workflow, map->adversarial-verify pipeline over 869 untraced defs + 924 dangling @traces sites (721 = phantom REQ-100-*). Ground truth staged from the authoritative specs: 4864-requirement index; packets exclude phase-5 in-flight files. Rules: no id outside the index, unmapped = spec-gap finding (never fabricate), check-traces + import gates per packet
- Phase 5/5b both parts delivered (uncommitted, interleaved with live tracing swarm): part A killed the ContextInjector._driver reach-through (pipeline_kg owns one repo), migrated context_pack+synchronizer+QueryMixin, deleted the _driver compat property; part B did kg_context/kg_sync-escape-hatch/fireflies_sync/canonical_schema. context_injector 81% = known retired-handler debt (P3 deletion backlog). Part A also caught+fixed llm/config.py missing traces import from the swarm; other 9 flagged files self-healed via swarm gates
- CP1 suite triage: fixed a real part-A bug (context_injector PEP-649 lazy annotation NameError — SyncGraphRepository import hoisted from TYPE_CHECKING to runtime), 3 signature tests updated driver->repo (intentional seam API), 2 placeholder-pinning tests rewritten to durable invariants (they asserted REQ-000-000 IS present — enshrined fabrication; now assert well-formed + placeholder-free), 3 missed TestResultsStore(driver=) constructors in test_real_neo4j_knowledge fixed. All 12 flagged modules green; full suite rerunning
- Runner defect FIXED and committed (ca50077f): root cause measured — i%48 round-robin packed 9 fast modules behind a 285s Neo4j-error-path module in batch 7, crossing the 273s graceful timeout; LPT packing from a duration cache + explicit did_not_run flag + one rerun round; 140 threading tests green, 98% file coverage. Runner-fix agent TaskStopped on acceptance
- Runner gap-2 fixed: modules SIGKILLed mid-test now rerun-eligible in SOLO bins
- live-log rescue keeps did_not_run+suspected_hang; _prepare_rerun_batches gives each suspected hang a solo bin so a real hang cannot poison shared rerun batches
- 53/53 executor tests pass; commit 32d48510
- Launched pytest-native-features audit swarm (wf_b332f148-faa)
- 6 research agents: core/reportlog, xdist deadlock forensics, pytest-split, rerun plugins, timeout/hang, free-threaded parallelism
- synthesis writes plans/pytest-native-audit.md
- pytest-native audit complete + adoptions committed (c7cffea6)
- 7-agent swarm -> plans/pytest-native-audit.md; adopted: -p no:xdist + no:cacheprovider per batch, faulthandler_timeout=per-test+30, pytest-timeout>=2.4.0 pin
- measured: grpc and janus_swi silently re-enable the GIL on 3.14t (isolated per-process check)
- W6 acceptance FAILED: 224/407 = 55.0% (gate 95%); 183 wrong decorators saved to plans/trace-acceptance-wrong.json
- judge quality verified by 2 direct spot-checks (peek/REQ-007-US-005-011 class D, handle_put/REQ-014-129 confirmed)
- root cause: W1-W5 were defect-class-driven; population mass never flagged by any detector retains fabricated ids
- worst strata: gateway 14.3%, kernel 29.5%, knowledge_graph 33.3%; best: harness/tools/openspec/refactor 100%, postman 94.7%
- W6.5 launched (wf_d37360aa-3ba): 154 fix+verify packets over 3,283 decorators, 18 failing strata
- packets grouped by file (386 files), each site carries top-8 OpenRouter RAG candidates; 200 sites carry their W6 judge verdicts
- brief: exhaustive per-id re-judgment, ~45% measured base rate, alias-class ruling added
- excluded strata (>=90% in W6): postman, harness, openspec, refactor, tools
- Suite hotspot analysis + fix: unbounded Neo4j managed-transaction retries (commit 873cee21)
- top-2 modules 438.5s -> 66.5s (76/76 pass); serial suite 52.9 min -> ~46.7 min
- mechanism: execute_read retries ServiceUnavailable for max_transaction_retry_time=30s default against dead addresses; bound to 1.0s
- remaining top: test_pipeline_kg 184.8s, test_orchestrata_tools 128.6s, test_coverage_queue_manager 103.4s, test_test_executor 83.3s (justified: spawns deliberately-hanging pytest subprocesses)
- Profiled the 3 slowest modules; fixed 2 (commits 917099b4, 9811dd51)
- test_pipeline_kg: 4th unbounded-Neo4j-retry instance, 100.7s -> 9.6s
- test_coverage_queue_manager: redis-py 7 default Retry(ExponentialWithJitterBackoff,3) = 5.3s/command vs dead port; QueueManager gained redis_client injection seam; 94.3s -> 2.1s
- test_orchestrata_tools: structural — 5 API entry points each re-run a full-codebase matching pass (~15s each); fix belongs in orchestrata_tools matcher memoization, deferred with numbers
- orchestrata_tools matcher memoization (commit ab192aa1): module 128.6s -> 20.5s
- identity-keyed bounded cache on (test_ast, source_index); 15.15s -> 0.03s cross-instance; copies returned, lock-guarded for 3.14t
- test fixtures rerouted through cached _source_index() singleton
- W8 prevention shipped (6bdd74ec): trace-lint spec-mismatch rule + deterministic provenance pass + pre-commit + conventions
- tree: trace-lint 0 errors, check-traces 0 dangling, 505/505 trace-asserting tests
- 2 tests updated that pinned pre-ruling state (alias-class, core/base infra-lane)
- Evidence-derived trace writer: citations→direct-call evidence→@traces
- add_traces_from_evidence.py joins REQ citations (docstring/comment only) to source defs via BodyMatcher.direct_call_index (new public API; fixture+decorator calls excluded, import-disambiguated); 25 new decorators + 18 merges across 22 src files, all membership-true, trace-lint 0 errors, 381 affected tests + 30 new tests green
- Full-gate pass over the evidence-trace change set; 2 real bugs fixed
- code-review.py tree-sitter path never ran (undefined _py_lang NameError swallowed); generate_rtm_evidence tokenize.TokenizeError->TokenError; mypy --strict 0 on changed files; TRACE_MISSING downgraded to WARNING + scoped off orchestrata_tools/scripts per two-lane ruling; suite 22795/0/0 in 336s
- orchestrata_ast extraction complete + BodyMatcher split steps 1-5
- New stdlib-only src/orchestrata_ast (models/analyzer/serialize/inheritance) extracted from the 9.6k-line script; script re-exports; serialization byte-identical; orchestrata_tools + evidence writer consume orchestrata_ast.inheritance; local InheritanceMap deleted per ruling 'no hand-rolled inheritance checks, enhance the AST component'
- Fixed parallel-suite flake: PipelineEventBus events_key seam
- test_emit_with_redis counted on the hardcoded shared Redis list; 8-way parallel batches interleave; seam added (default pinned by test), fixture docstring's 'suite runs serially' premise corrected; committed
- Root-caused coverage combine data loss: relative COVERAGE_FILE resolved against grandchild cwd (tmp_path) — parts deleted with fixture dirs. Fixed with absolute anchor (b2a95525); relaunched instrumented full suite for a true baseline.
- All 16 zero-percent files confirmed measurement artifacts (probes 78-100% real)
- User directive: coverage waves until every src file >90%
- ROOT-CAUSED + FIXED the whole-batch coverage loss (bd88c205): conftest pytest_unconfigure's os._exit grpc-hang workaround skipped atexit, where patch-mode coverage saves parts — every grpc.aio-importing batch exited green with zero coverage. Fix: stop+save Coverage.current() before hard exit. Verified via PTE_COV_PART_LOG diagnostic (26/60 children parts=0 → killers now parts=1).
- Also isolated nested pytest-cov data files in test_session_coverage_gates (latent slow-lane part destroyer)
- Coverage source expanded to orchestrata_ast + orchestrata_tools + scripts (8dcbc9f9) per user ruling 'not just src'
- Run 4 (clean baseline) in flight
- Coverage wave 1 committed (94d6fc3e): ultimate+ast+tools all targets 92-100%; 628 tests green combined; 10+ real source bugs fixed incl. @traces propagation that NEVER worked in enhanced_generator, classmethod/async codegen bugs in diagram_to_code, runtime_main serve seam. Dead-scripts deletion committed (6a186c9d, 29 files ~6k stmts, user-approved). Wave 2 (5 agents) running on live scripts.
- src/orchestrata_ultimate truly 97.8% — the 156-file worklist was the os._exit measurement bug
- scripts audit: 35 live to test, 14 deleted, 4 unwired ruled TEST
- Wave-3b/3c committed: CLI wrappers + ops/gate scripts >=90%
- 8d6aa61f: rewired test-gen.py/review-tools.py (both crashed on every run — API drift), ast_api.py Path fix, 78 CLI tests
- f7a64c9f: agent_check mypy --output-format=json silent-zero bug + auto_fix LineLengthFixer discarded-split bug fixed, 132 tests, cluster 99%
- MemgraphSemanticGraph.clear() Entity-wipe removed per user ruling (uncommitted, rides with semantic-graph cluster)
- In flight: runner cluster, semantic-graph ingest >=90%
- Gate to 0 blocking + PYTEST_XDIST_WORKER renamed to ORCHESTRATA_WORKER_SLOT
- d50b2a95: gate-ok waiver channel (reason required) for 11 fixture-string false positives + 1 label-scoped delete; gate now exits 0
- 2aee8349: env var renamed repo-wide (user ruling — old name read as xdist); gw<N> format kept; agent defs, hook, docs, memories updated
- db4cf1c8/5f2d5ef4: runner + semantic-graph clusters landed; ingest 100%, clear() Entity-wipe removed
- Final instrumented full-suite run in progress
- Seam #163 phase 6 (final): D13 graph-seam gate committed (219b83c3)
- AST gate: neo4j imports + .session( calls banned outside seam/escape-hatch allowlist; probe pattern (bare import in try/except ImportError) sanctioned
- Caught 4 probe imports the grep inventory missed; checker green on 650 files; 11 tests, checker 97%
- Also: force-test-observation deny-hook (8b791903) + memory; wave-5 sweep found 11 files <90%, 4 test-writer agents in flight
- Rogue auto-commit 724b556e ('Session 400: Say hello...') reverted
- An external session's auto-commit swept 18 uncommitted in-flight files (finished wave-5c work + two agents' half-done files) into one commit
- git reset --mixed HEAD~1 restored the exact pre-commit tree; daemon roster stale, no live rogue process found
- Wave clusters recommitting individually after verification as planned
- Solo-retry round in run_tests.py + executor JUnit-attribution fix
- Capped solo-retry (default 5, --solo-retry-cap) re-runs batch-failed modules in isolation; both rounds reported, recoveries logged to .flake_recoveries.ndjson; exit reflects post-retry verdict
- Real executor bug found via the fixture: _dotted_module_map garbled absolute paths so whole batches read as did-not-run and double-ran via Phase-2B — fixed (relpath to cwd; out-of-tree = stem) + normalized file-attr matching
- force-test-observation hook runner regex tightened twice; 23-case matrix green
- Commits 4916e57a, fec5926b
- Drafted plans/reproducibility-plan.md (Task #9): live fleet audit + 6-phase plan
- Audited 23 Incus containers: redis-queue .10, neo4j-db .11 and ALL Memgraph HA nodes are DHCP-lease-only (no ipv4.address pin) — the load-bearing IPs are the unpinned ones; only 8/23 pinned
- Memgraph MAIN is .111 (failover from .192) — plan mandates resolve_main() via HA controller, never doc-pinned MAIN
- Phases R1 pins, R2 .env.example+validate_env, R3 MAIN discovery, R4 bootstrap_graphs (kg_schema+sync_ast+sync_architecture+enrich), R5 Terraform+incus provider with import-not-recreate + drift gate + rehearsal project, R6 CI parity via canonical runner; acceptance criteria + effort table
- RELEASE v1.3.0 pushed — session goal achieved
- master bc4edb87 (107 commits) + annotated tag v1.3.0 pushed via glab helper; verified on remote via ls-remote
- v1.2.0 was found already published at old commit 89bf5d11 (premature tag from an earlier session) — published tags never moved, released as v1.3.0 instead
- Coverage evidence: no-filter sweep 97.6% total / 74151 stmts, all first-party files >=90%; llm_integration-gated scripts measured explicitly (93%/94%) since default addopts -m excludes them from every sweep
- Solo-retry runner proved itself live: 3 full runs, each with 1-2 batch flakes, all auto-recovered solo, flake log accumulating (test_constraints_real.py now a 2x repeat offender)
- Memgraph→orchgraph migration: safe prep done, gated on kimi + sign-off
- Backup: 558M verified tar of source data dir. Discovery: source cluster shared (762376 nodes/497961 rels, 58 labels, orchestrata + knowledge-wiki co-mingled)
- IaC committed f2d49bc8: terraform+incus for orchgraph, validates clean, hardens the two live-cluster defects (data-dir /tmp->/var/lib persistent; DHCP->pinned IPs)
- Parity verifier built + baselined (the pre-decommission gate). Mohamed decision: full port both projects + decommission old, AFTER parity PASS + sign-off
- BLOCKED on kimi Step 1 (move orchgraph data-dir off /tmp) then Step 2 (snapshot/DUMP port); kimi async, only canned acks so far
- Built and deployed vector_search.so to orchgraph Memgraph HA cluster; unblocked RAG by enabling vector_search.search/search_edges/show_index_info procedures.
- Diagnosed root cause: orchgraph fork binary lacked query-modules-directory config and vector_search.so deployment; systemd units updated on orchgraph-data-1 and orchgraph-data-2.
Artifacts
- plans/neo4j-seam-design.md
- plans/trace-remediation-plan.md (190 lines) - Trace remediation program: defect taxonomy D1-D9 (5061 decorators/14395 ids; fan-out REQ-002-001 x370; 199 shotgun decorators; 196 sequential-stamped files), waves W0-W9 with entry/exit criteria, statistical acceptance gate, downstream rebuild, prevention gates
- plans/bodymatcher-split-plan.md (170 lines)
- plans/prolog-utilization-plan.md
- scripts/run_tests.py
- src/orchestrata_ultimate/core/threading/test_executor.py
- plans/reproducibility-plan.md
- /home/mohamed/workspace/orchgraph/build/query_modules/vector_search.so - Built query module for orchgraph Memgraph 3.11 fork (Debug+MG_ENTERPRISE=ON)
- /tmp/deploy_vector_search.sh - Deployment script that pushes vector_search.so and updates systemd units on orchgraph data nodes
Decisions
- Trace remediation runs as a staged program (plans/trace-remediation-plan.md), not ad-hoc rounds: User ruling: rounds 1-2 were reactive; census showed the real problem ~10x larger (default-stamping top-5 = 1371 occurrences, 199 shotgun, 196 sequential files) — needs instrumentation, serialized waves, fresh-eyes statistical acceptance, artifact rebuild, prevention gates
Watch items recorded in plans/pytest-native-audit.md
- Two-lane traceability: business code -> FRs; infra code -> explicit NFR or deliberately untraced: User ruling 2026-07-09: infra serves many purposes; force-fitting business reqs onto utilities is the fabrication engine (RingBuffer.peek/autonomy-verification). Codified in plan §2.5 + live w65_brief.md; W9 must separate untraced-infra(by design) from untraced-business(spec gap)
- Trace program FROZEN at prevention-only after strategy review; W6.5 stays stopped: W1-W5 wave QA (0.6-12.5% wrong) vs W6 corpus measurement (45% wrong) = non-convergence; 95% gate set without measuring inter-judge agreement; no named consumer of the corrected corpus. Deterministic replacements shipped in 6bdd74ec: spec-mismatch lint rule (found on 55% of judged-wrong decorators), 118-decorator provenance normalization (15 rewrites + 103 splits), pre-commit gate, AGENTS.md conventions, 55%+/-5% corpus caveat
- check-traces ratchet re-baselined 236→2219: code_reqs dropped 4654→2636 distinct ids via deliberate W4/W5/W6.5 removal of measured-wrong claims (-1331 pre-b4ae35e5, -687 in b4ae35e5); old floor rested on 55%-wrong decorators; 2219 untraced is the honest count
- Deployed /opt/memgraph/query_modules/ instead of /usr/lib/memgraph/query_modules because orchgraph binary lives at /opt/memgraph/memgraph: Container inspection showed /usr/lib/memgraph does not exist; /opt/memgraph is the actual deployment prefix.
- Built vector_search.so in existing /home/mohamed/workspace/orchgraph/build tree: Claude verified deployed /opt/memgraph/memgraph is byte-identical to build/memgraph, so ABI/config match is guaranteed.
