How does the project enforce the rule against bare exception handlers?

The rule against except: pass and except Exception: pass is enforced in two places:

  1. Static review: src/orchestrata_ultimate/verification/review/checks_structure.py::check_error_handling performs an AST scan and reports any bare except: or except Exception: clause that does not log or re-raise the exception.
  2. Agent review: the CodeDirector volk runs the same structural check and rejects any patch that introduces a silent exception handler.

Every exception handler must log the actual error or re-raise it. Silent failures are banned because they are the hardest bugs to diagnose.
